New Patient Registration Process

At this time, we are not accepting new patients with Oregon Health Plan (OHP) or PacificSource Community Solutions due to temporary clinic capacity limitations. Established patients continue to receive care.

Please Review All Steps Before Submitting a New Patient Appointment Request

Step 1: Let’s Begin
We are thrilled that you're ready to start your journey with us! Begin by completing our New Patient Appointment Request Form. This is your chance to tell us about your needs, whether for yourself or your child. Rest assured, your information will be handled with the utmost care and confidentiality.

Step 2: Our Team Reviews New Patient Inquiries
Once your form is submitted, our clinical team will carefully review your information and may reach out with additional questions. We aim to address all New Patient Appointment Requests within 5-7 business days, although this may vary based on the volume of inquiries. If we determine that we are a good fit, we will contact you to guide you through the next steps of our registration process.

Step 3: Complete Your New Patient Registration Forms
As part of the registration process, you are required to complete a New Patient Registration Packet through our patient portal called MIYO. It is essential that your MIYO account is activated and all required forms are completed before an appointment can be scheduled. Upon logging into MIYO, you can locate these forms under "Clinical Forms." The packet includes a patient health history form, payment and insurance authorization forms, practice policies and agreements, and measurements.

Step 4: Stay Connected with Spruce
We use Spruce Health, a secure HIPAA-compliant messaging platform, to communicate with our patients. For easy access and streamlined communication with our practice, we recommend downloading the Spruce app on your mobile device. Step 5: Registration Review & Verification
Once all required registration forms have been completed in MIYO and your Spruce connection has been established, our team will review your paperwork for completeness. We will verify insurance eligibility, review your submitted documentation, and ensure all required forms have been received. If additional information is needed, a member of our team will contact you through Spruce before scheduling can proceed.

Step 6: Schedule Your Evaluation
Once we have reviewed your completed registration paperwork, we will be ready to schedule your initial evaluation. We will contact you via text message through the Spruce Health app to find a time that suits your schedule and our earliest availability. Please note that appointments are only offered to patients who have completed all required registration steps.

Scheduling Appointments: New patients, whether adults or minors, are typically scheduled within 3–5 weeks after all required paperwork has been completed and insurance/payment information has been verified. Please do not use the patient portal to schedule new patient appointments, as these requests will be denied. The portal is reserved for established patients who have completed their initial evaluation.

Need Help Accessing Your Forms?

Step 1: Create Your MYIO Account
Use the link provided in your MYIO invitation email or text message to create your account. If your invitation has expired or you have not received one, please contact our office for assistance.

Step 2: Verify Your Email Address
Follow the prompts to verify your email address and complete your account setup. Your email address will be used for password resets, secure messages, and important updates regarding your care.

Step 3: Complete Your Registration Forms
Once logged into MYIO, navigate to Clinical Forms and complete all required registration paperwork. This may include health history forms, consent forms, insurance information, practice policies, and other registration documents.

Step 4: Wait for Scheduling
After your forms have been completed and reviewed, a member of our team will contact you through Spruce to schedule your initial evaluation based on provider availability.
